11/2/2023 EnchantaVaughn Approved Behavior: QMHPA arrived to the home of Ms. Broadnax. Upon arrival to the home setting, Ms. Broadnax appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHPA inquired about her day. Ms. Broadnax indicated her day was well. Ms. Broadnax was well groomed and prepared for the session. Intervention: QMHPA began the session by inquiring about Ms. Broadnax’s current thoughts, feelings, and emotions. QMHPA reminded Ms. Broadnax the importance of critical thinking, active listening, self- monitoring and journaling as tools she can use to redirect behaviors and behavioral responses, she can use 5 times per week. QMHPA used role-playing and role-reversal exercises to help Ms. Broadnax identify appropriate verbal and non-verbal responses she can use when she is at work. QMHPA reinforces reducing depressive symptoms with daily exercise and deep breathing to help her relax. QMHPA helped Ms. Broadnax implement a schedule she can live with that introduces a daily exercise. This skill will help Ms. Broadnax decrease periods of depression through her concentrated efforts in exercising and journaling daily. QMHPA reminds Ms. Broadnax to begin identifying stressors associated with her depressive thinking. QMHPA assisted Ms. Broadnax with identifying and listing those stressors that causes depressive symptoms. Ms. Broadnax was also encouraged to use her journal as a coping tool. QMHPA assisted Ms. Broadnax with identifying triggers associated with being depressed. Response: Ms. Broadnax appeared to be receptive to QMHPA’s interventions and services that were provided to her. Ms. Broadnax shared that it seems like she will always be depressed, anxious, nervous and on edge. Ms. Broadnax was able to identify three stressors that she stated continues to plague her life. Ms. Broadnax stated that she is often faced with financial stresses, boredom, being employed at a place that makes her feel depressed 3-4 times per week. Ms. Broadnax was able to identify some areas that she feels she has made progress with. Ms. Broadnax verbalized to QMHPA she continues to need to just focus on the bigger things in life and not the little things that she does not have control over. She expressed to QMHPA on how she was ashamed about the disease she has; however, she cannot let that get her down. At the end of the session, Ms. Broadnax expressed to QMHPA she was going to take her medication as prescribed and continue to really follow up with her PCP regularly. Plan: QMHPA will continue to teach Ms. Broadnax how to identify as well as process triggers and educate her regarding positive coping skills and interpersonal skills 1-2 times (Weekly). QMHPA will continue to teach Ms. Broadnax how to schedule as well as manage appointments and educate her regarding the positive impact of medication through various skill building activities 4-5 times (Monthly).
